Medicina defensiva

Acabo de salir de la UCI de neonatología de un hospital español cualquiera. Acabo de encontrar a unos padres agotados o más bien exhaustos. Unos padres valientes que tienen la mala suerte de enfrentarse a uno de los peores problemas que alguien puede tener y a lo que les deseo la mejor suerte del mundo. 

Hay quienes dicen que mi forma de hacer medicina es "defensiva". Lo dicen porque comienzo poniéndome en la peor de las opciones posibles para el paciente y descartando desde ahí. Lo hago así porque normalmente es más difícil que te pases cosas por alto. Pero, sobre todo, lo hago así porque no me gusta correr riesgos, no me gusta someter a mis pacientes a un riesgo innecesario. Nunca asumo nada si no tengo pruebas. 

No es medicina defensiva, es no querer tener que ver a unos padres que pasan una tragedia por algo que puede (o no) ser evitable. 

Básicamente es poder dormir tranquila. Y si, es defensiva lo

es porque defiendo de la forma que mejor sé los intereses de mis pacientes. 

Pista al caso clínico anterior

Al realizar la artroscopia de rodilla el traumatólogo se encuentra con una imagen que coincide con el cuadro clínico: las superficies articulares también se encuentran teñidas de negro.

Paciente varón de 35 años que acude a consulta por dolor articular de 3 semanas de duración, especialmente en la rodilla derecha, aunque refiere molestias en ocasiones previas. Como antecedentes médicos presenta historia de lumbalgia. La Tª es 36,5ºC, TA 115/85, FR 14 RPM, Sat 98%.

Como parte del estudio se le pide analítica de sangre y orina. Mientras el bote de orina está en la bandeja para ser transportado a laboratorio, cambia de color a negro.

¿Cuál es el diagnóstico más probable?

Hace unos años cree una hoja de cálculo que me permitía saber cuanto debían pagarme en cada nómina en función de las guardias que hiciera.

La acabo de reencontrar por aquí, y he pensado que sería buena idea compartirla con todos Vds. Para saber el sueldo final sólo tienen que introducir el número de guardias laborables (lunes a viernes) o festivas (sábados, domingos y fiestas de guardar) según el año de residencia en que se encuentren. Además al final hay una columna deben agregar el IRPF que les descuenten.

Las guardias "especiales" hasta donde yo supe eran: Nochebuena, Navidad, Nochevieja, Año Nuevo y el día de Reyes.

Let us look over the hatred

I am sorry but I do not buy it. It is taking a long time to convince people that all of these massive killings have something to do with religion. I would never buy such stupid idea. Hopefully many people are on the same page. As most Spaniards, I was educated as christian, and I do believe is a good religion to have faith in but I do not think that It is better than others. 

Fortunately, I have had the chance of meeting people all over the world, with all kind of different beliefs, colors, religions and cultures.  I have to say that not every one of them has been nice to me. Anyways, I have met good and nasty people all over the world. If you ask me, I would say that being mean, is more related with a low level of education than with any other of those factors that you are trying to sell me. Actually, to be fair enough, I ought to say that some of the most miserable people I have ever met came from my same cultural baggage. 

So If you really think this has something to do with being muslim or not, I can not care less, but please keep out of my way. I strongly ask to anyone who is reading this to NOT target anyone for having a different color, way of living or religion. At the end of the day, we all want the same, that is mainly being happy. Let us be enough open minded to see over this hatred that they are trying us to buy.
